Optimized Needle Biopsy Strategies for Prostate Cancer Detection
نویسندگان
چکیده
Clinical diagnosis of prostate cancer is most often done by transrectal ultrasound-guided needle biopsy. Because of the low resolution of ultrasound, however, the urologist cannot usually distinguish between cancerous and healthy tissue. Therefore, most biopsies follow standard procedures (known as “protocols”) based on long-term physician experience. Recent studies indicate that these protocols may have a significant rate of false negative diagnoses. This research develops optimized biopsy protocols. We use real prostate specimens removed by prostatectomy to develop a 3D distribution map of cancer in the prostate. We develop also a probability model of the needle insertion procedure. Using this model, the tumor map, and the geometry of the biopsy needle, we obtain estimates for the probability of obtaining a positive biopsy in various zones of prostates with cancer. Using this we develop a nonlinear optimization problem that determines the protocols that maximize the probability of cancer detection for a given number of needles, and present new optimized protocols.

Background: Transrectal ultrasonography guided needle biopsy of prostate frequently used for early detection of cancer has faced the pathologists with a major diagnostic challenge. In recent years P504S has been used as a tumor cell marker for definitive diagnosis of prostatic cancer in small biopsy specimens. Methods: 70 prostate needle biopsies and 6 transurethral resections (TURP) containing ...

Background & Objective: As the prostate adenocarcinoma is one of the most common malignant tumors in males, looking for a marker to effectively predict aggressiveness and metastatic potential in an apparently localized cancer in initial needle biopsy specimens can help the clinicians to make more appropriate decision for treatment, planning, and choosing appropriate ta...

متن کاملA Statistical Atlas of Prostate Cancer for Optimal Biopsy
This paper presents a methodology of creating a statistical atlas of spatial distribution of prostate cancer from a large patient cohort, and uses it for designing optimal needle biopsy strategies. In order to remove inter-individual morphological variability and determine the true variability in cancer position, an adaptive-focus deformable model (AFDM) is used to register and normalize prosta...

Background: Diffusion-weighted imaging (DWI) is a main component of multiparametric MRI for prostate cancer detection. Recently, high b value DWI has gained more attention because of its capability for tumor characterization. Objective: To assess based on histopathological findings of transrectal ultrasound (TRUS)-guided prostate biopsy as a reference, an...
